To make the application process as easy and fast as possible, the credit application document is available here for downloading and printing. Simply right-click on the graphic at left and then save and print the application form. Please thoroughly complete the form and mail or fax it to the Accounts Receivable Department at our Springfield, Illinois office.

If your operation is Sales Tax exempt, please include the sales tax exempt forms applicable to your tax jurisdiction. Simply select the applicable state from the drop-down list below and complete the form to submit along with your credit application. Should you find an incompatibility with your software or operating system, please notify us and we will be happy to mail the forms to you.
